Nurse Practitioner – In-Home Care
About the Role
We are hiring Nurse Practitioners to deliver acute-level care in patients’ homes throughout Palm Beach county. This model blends the responsiveness of urgent care with the comfort and impact of house calls.
This is a unique opportunity for Nurse Practitioners who are confident practicing independently and passionate about transforming care for medically complex and underserved populations.
Key Responsibilities
- Respond to unscheduled referrals and perform urgent, in-home visits
- Conduct clinical assessments and focused physical exams
- Administer treatments such as IV fluids, wound care, splinting, and injections
- Order and interpret labs, EKGs, and bedside diagnostics (e.g., point-of-care ultrasound)
- Coordinate with referring providers and care managers to ensure continuity of care
- Educate patients and caregivers with a palliative, patient-centered approach
Qualifications
- Active FL Nurse Practitioner license
- Ability to work independently in a fast-paced, field-based role
- Strong clinical judgment, communication, and adaptability
